You must be eligible for Medicaid. CDPAP is funded by Medicaid,so you must be eligible for Medicaid in New York to be eligible for CDPAP. This usually means you need to have a low income and limited assets. You need to request home help services: the CDPAP is aimed specifically at people who need home help services, such as help with acts of daily living, medical tasks and other tasks related to maintaining independence at home. You must be able to direct your care yourself: The program is called consumer-directed because the person receiving care is responsible for hiring and managing their own personal assistant.This means you must be able to make decisions about your care and direct your personal assistant on how to provide that care. You must have an eligible condition: To qualify for CDPAP, you must have an eligible medical condition that requires home care services. This can include chronic illnesses, physical disabilities, or cognitive impairments. If you meet these criteria, you may qualify for CDPAP in New York.
